NFTY - Reform Jewish Teens - NFTY Health and Safety Form

A signed and completed health and safety form is required to be on file with the NFTY-CWR regional advisor for all participants. NEW THIS YEAR: this form can now be submitted annually (NFTY year runs from June 2011 - July 15, 2012) and updated as needed for future events during the year.

Please note that this form is different from the medical form submitted for the URJ Camps, NFTY in Israel programs, NFTY-EIE High School in Israel and Mitzvah Corps and therefore a NFTY form is still required for those teens who attended those programs this past summer.

I AM SUBMITTING A NEW HEALTH FORM:
This is my first event of the 2011-12 NFTY Year. I will be submitting a signed and completed NFTY Health and Safety Form A.

Remember that signatures of the participant, a parent/guardian and rabbi/cantor/educator/advisor from your congregation are needed.
Forms can be mailed, sent by fax or scanned in an e-mail as an attachment (see below).
Your registration for this event will not be considered complete until this form is received. Send your signed and completed form by mail within 7 days of registering for the event. If you are not able, please e-mail sleader@urj.org.

MY HEALTH FORM NEEDS UPDATING:
I have already attended an event during the 2011-12 NFTY year, however some information I submitted on my NFTY Health and Safety Form has changed. I will be submitting an signed and completed Health and Safety Update Form B.
